I would like to know a few more details. Your average blood glucose for 15 days if you are monitoring or if you have a recent a1c. What is the most you ever weighed and what is your current weight ? BMI?
Did you have history of pancreatitis or pancreatic surgery?
Do you or did you drink alcohol excessively?
Is there any close family member with type 1 diabetes? Or lean type 2 diabetes?
Do you have autoimmune disease such as thyroid problems, rheumatoid arthritis, adrenal dysfunction ? Etc.
I want to make sure you have accurately classified diabetes because quite a great proportion of patients given a diagnosis of type 2 actually have late onset autoimmune diabetes of adults ( LADA) and these patients need exclusive insulin treatment to manage their diabetes. Quite often even for type 2 with acute hyperglycemic burden , we use insulin briefly . We can slowly transition to non -insulin options if it is confirmed that it is type 2.

Symptoms historically and traditionally associated with thyroid dysfunction are notoriously non-specific including these symptoms you shared. They could be due to entirely different reason than the thyroid. The good news is that nowadays, it is not difficult to tell or diagnose if you have thyroid dysfunction. A simple blood test will solve that. If the blood test not show it , you do not have it. In primary care setting they usually test TSH which can be enough in 90% of the times. Higher TSH ( above normal range) shows underactive thyroid (hypothyroidism- which needs thyroid hormone supplement or replacement), low TSH ( below than the normal range- which would indicate overactive thyroid or Hyperthyroidism- which needs treatment to slow the thyroid down). Endocrinologists usually do a more completes test on the thyroid (TSH, Free T4, Free T3, thyroid antibodies) for 100 % diagnostic accuracy and to determine reasons behind thyroid dysfunction. Goiter is enlarged thyroid- which may need thyroid ultrasound for better assessment. So, I suggest you get assessed by either PCP or endocrinologist to answer this important question you asked. The liver is making (emergency fuel) glucose when you go relatively down overnight. Wearing a continuous glucose monitor such as freestyle Libre or Dexcom even for a brief period of time will help you pinpoint the issue. That is why you see a higher glucose in the AM than PM. Your care must involve measures to minimize the hyperinsulinemia. When it comes to insulin, the less, the better specially if the duration of diabetes is less than 10-15 years.
