If he placed a lot of water/air and he has some pelvic distension, then he needs further work up. The most important thing is to try to prevent this from happening again. It is not good to put anything in the urethra unless medically necessary. Talk to him and try to find out why he did it.

If you have scrotal edema, the best thing is take your diuretics as directed by your physician and keep your scrotum elevated.

It can mean a number of things including but not limited to : kidney stones, infections, kidney diseases, cancer, etc. If you have blood in the urine it must be worked up.
