Not all implants require bone grafts. Majority of the time when a tooth is extracted we bone graft for socket preservation and implant placement in 4 months. If a sinus has pneumatized, a sinus lift can often be performed at the time of implant placement. If a tooth has been missing for a long time, and there is significant bone loss, this often necessitates more complex bone grafting called Ridge Augmentation.

Typically, an experienced oral surgeon will numb the area with a local anesthetic and extract the tooth. The local anesthetic blocks the pain fibers, but it is normal to feel pressure and pushing. However, if you have an infected or “hot tooth” that has been hurting for a long time, it may be more difficult to numb the tooth. Depending on a patients medical history, this may warrant office sedation for patient comfort without any pain.
Hope that answers your question.
