I assume that you’re referring to an inguinal (groin) hernia, but there are several types of inguinal hernias, also. Indirect is the most common, occurring along the spermatic cord exit from abdomen. However, direct is quite common and is a bulge medial to the inferior epigástric vessels. Also, varies with age, body habitus, comorbid conditions, etc

Right lower abdomen, but can be right flank, groin, mid-belly.

Generally, not very painful, if done laparoscopically. But, if situation demands open surgery, it HURTS!
