24/7 extensive Right Hip groin pain- Q for any orthopaedic hip surgeon or hip specialist .

If a person is still in 24/7 agony in right hip specific to groin area- cracking/soreness/swelling for 9 years worsened to the point now cannot walk
Recent MRI done- despite a 'normal' MRI - dr says MRI is reassuring and shows no grossly concerning features regarding the joint itself.... is hip arthroscopy ever an option? or is it just physio?
Patient has been discharged and palmed off to physio- however I thought further investigations would be done such as biopsies, more imaging and further investigations even arthoscopies before such conclusion was made?

This problem has occured for a long time- and exercises have been tried before (across years) with no relief.

Are MRIS always 100% used to indicate whether arthroscopy should b used?
Or are hip arthoscopies used in cases where patient has complaint yet MRI shows ok joint? do MRIs show everything?

Could the groin pain be from anywhere else but the hip/ such as spine?

May need an MRI Arthrogram
Groin pain can be referred from other areas like the spine or sports hernia, or athletic pubalgia. Good clinical evaluation is most important. If the hip MRI is normal, then you need further evaluation like for the spine or pelvic area. Sometimes if the MRI is normal and spine and other causes of pain have been ruled out, your doctor might order an MRI with dye. If MRI is normal then I would not suggest any hip arthroscopy. Diagnostic lidocaine injection in hip joint is important to determine if the source of pain is hip joint or extraarticular like spine or torch bursitis. I hope it is helpful. Thanks
