Cardiac? or being a Hypochondriac?

Male, age 31, 5 foot 6 inches @ 220 pounds, taking 10mg Lisinopril.
Noticed left side chest tightness and aches, back upper pain and pain appearing in the left armpit area during random times of exertion. Concerned this is Angina.
Have high cholesterol, CPTSD and lots of anxiety as well.
This only started a few weeks ago, however noticed said symptoms appear more intensely when under tremendous mental stress at work.
When resting, symptoms seem to disappear and go away.
Haven't noticed any breathlessness or dizziness at all.
Had went earlier to the gym today and other times, swinging kettlebells for about 30-40 minutes for good exercise, and haven't noticed any of these issues appearing. Confused and concerned whether this is just beginning or an onset to a bigger cardiac issue or if i'm just building it up in my mind?

Last time I went for a full heart checkup was about 1-2 years ago, and was cleared, no issues and no problems.
For this whole past year, was not eating consistently, due to fatigue and lethargy which stemmed from low Vitamin B12 and Vitamin D. Taking those supplements now and feeling better, however do wonder about the cardio aspect.

You have hypertension and untreated hypercholesterolemia, with admittedly a poor diet and you have significant stress so there are certainly cardiac risk factors here. While your symptoms are only intermittently associated with physical exertion, its not unreasonable to screen you with a stress test, particularly since going to the gym would likely have aggravated musculoskeletal pain and it didnt seem to do so. You have both typical and atypical features of your symptoms, so further evaluation should help provide you piece of mind if not an early diagnosis.
