“Discharge plan for ckd ”
Good day!,Hello po Doc These questions are intended for my case presentation po, since it's hard to find out those plans here on the net. That's why I came to this site, hoping it would answer my question. I just want to ask what those discharge plans are for CKD patients with a poor prognosis po?

Discharge planning is always important. The considerations must take into account the level of kidney function. The more significant the kidney disease the more detailed follow up is needed. Generally speaking though, discharge planning should incorporate evaluations of medications, diet, lab and clinical follow up with the nephrologist in a timely manner.
