Genital advice?

Red and itchy on testes. This has been going on since I was 16. Before I was sexually active. I want to know do I have crabs? Is this an std? Or just dry skin? It itches even after I have moisturized it

A skin biopsy is in order. My best guess is eczema. In any case a urologist can tell you (or a dermatologist). Doubtful as to crabs or STD
Doesn’t look like scabies, but could be due to the detergent or fabric of your underwear. Change the type of washing machine soap and use cotton underwear. If that does not work, see a dermatologist.

However, based on the information described, clinically, the skin change could represent chronic ezcema with some skin fungal infection. To manage such, it is worthwhile to try the followings: 1. keep the area as clean and as dry as possible; 2. watch it if some soaps or detergents induce such skin changes; 3. try applying topical hydrocortisone 0.5%-1% with antifungal agent thinly once or twice daily for 5-7 days, and see. If not cleared up or improved, see your doctor for further suggestions - like seeing a dermatologist or urologist. Combining all above instructions, you will be ready to provide a reasonable quality of self-care before seeing a doctor.

Not likely an STD. At this point, I would recommend you see a dermatologist. Seems to be a chronic dermatitis.

Looks like a skin irritation which should respond to topical hydrocortisone cream. Once you apply the cream, wrap that portion of your penis with saran wrap to prevent the cream from being absorbed by your clothes. Continue this daily as long as is necessary.
