Have I been infected with an STD?

I am a 32 year old male, I had protected sex with a lady who happens to be the only woman I've had sex with after 11 years of me abstaining from sex. This happened in late August 2021. I am having an itchy back and a very tiny nonitchy rash appeared on my shoulder. I'm also having an itchy palm. I've been depressed ever since I discovered that this girl is always scratching her body and I fear that I might have been infected with HIV. I used a condom and my sperm was intact inside it. Though the second condom I wanted to use, I didn't wear it well, I almost used it before she told me to wear it well. So I discovered I turned the wrong side of the condom, trying to wear it well, I then turned the other side of the condom to wear it well. Where did I go wrong?

Not sure that I understand your use of two condoms. Some people wear two condoms, one on top of the other. But it seems you used one and then replaced for a second episode/continued sexual intercourse. Also, you do not mention the type of contact: vaginal? Rectal? Oral? Your symptoms do not suggest any of the sexually transmitted infections. Not HIV, not syphilis, not chlamydia, not gonorrhea, not HPV, not Herpes, and not infestation such as pediculosis (crabs) nor scabies. Could be a tinea (fungus infection). So, it does not seem that you did anything wrong. Itchiness is a symptom that is often worsened by anxiety, and nothing more. To remove all worries, I suggest that you see a doctor or clinic that deals with STDs.
Hope this is helpful.
