Feel right away
I use laser to treat that kind of cavity. Drilling the tooth will create lots of damage to the enamel, that can’t be seen or felt for a long time, like micro fractures. They weaken the tooth and causing problems down the road. Find a dentist that uses a hard tissue(tooth) laser to protect your teeth for the long term. Teeth should last for life!
If it is truly a "cavity" ie., there is decay and the dentist's explorer (pick) can penetrate the enamel, then it should get a filling. If it is merely a stained groove, then watchful waiting is usually the best choice. It is a matter of the treating dentist's judgement.
If your pit and fissure cavity is a true cavity, it will need to be cleaned and a filling will be placed. You can have what is called "incipient" tooth decay, which is a very early form of decay that can be arrested or stopped from getting deeper. Theoretically, good oral hygiene can shift the balance in favour of remineralization and strengthening of the early decay lesion. This means you can potentially avoid the need for dental treatment!

If, however, the tooth decay has reached the layer underneath your enamel (this is known as "dentin"), it is at an irreversible point and will progress without treatment. These cases require removal of decay and restoration with dental fillings.
A pit and fissue cavity is a cavity on the biting surface of the tooth, or in a groove of the tooth. It is repaired the same way as any other cavity. The dentist will open the tooth to remove the damaged portion and then use filling material to fill in the hole.
