I don't know what skin problem this is and I need advice?

My mum has had this problem with her hands. I'm not sure if it's eczema, her hand is red and she has a lot of cuts on her hand and it bleeds. My mum says it itches very bad and it worsens at night. She has gone to the doctor numerous times, however, the creams the doctors provide for her do not help at all.

This problem sounds like hand eczema. The inflammation causes both abnormal loss of the skin barrier at the surface and itching that results in scratching that further damages the skin and causes even more inflammation.
