“I have coughing and choking in sleep with trach hole in my throat?”
In my sleep, I wheeze and cough, and choke badly and I have a tracheal hole in my throat.

You have a permanent tracheostomy? Symptoms appear to be aspiration-related, i.e secretions getting into larynx and trachea, which is triggering airway protective reflexes. See you internist or pulmonologist. Elevated head of bed may help, No late meals. Omeprazole may relieve reflux symptoms. No smoking. Limit caffeine intake.
