Kidney transplant medication?

Patient is a kidney transplant recipient in 2017. Inconsistent in taking medication, several days of symptoms: cold sweats, chills, fever, diarrhea, nausea, shakes, vomiting, unable to hold down solid, swelling in feet. Unsure if this is due to inconsistent medication or something else. Says he's fine. Which he is not.

Need to be evaluated ASAP. Please go to ER ASAP.
For above symptoms, see your transplant nephrologist. Advised to be compliant with your meds, otherwise you will lose your kidney and back on dialysis.

Could be CMV infection, kidney transplant rejection, UTI - I would take the patient to the ER for full work-up.
