You most likely have a hiatal hernia causing shortness of breath when you lean forward to tie your shoes. It’s caused by the stomach partially sliding up into the chest cavity due to a weakness of the diaphragm separating the chest cavity from the abdominal cavity. Other causes of your symptoms could just be related to weight gain and increased girth of your abdomen, however, less commonly there can be a fluid build up in the abdomen called ascites which can be caused by a serious underlying problem. Therefore, an evaluation by a medical provider is important to differentiate between these various causes and provide you an accurate diagnosis.
