MRI does not tell anything about dementia or PD.
I'm not sure what your question is. Looking at your MRI scans of 2012 and 2022 there is apparent atrophy of your brain. Although you can see this with advanced Parkinson's in a person your age there are concerns for other degenerative, dementing problems. I would suggest looking into a  specialized Movement or Memmory disorder center for indepth and detailed evaluation  diagnosis and treatment. https://www.hopkinsmedicine.org/health/conditions-and-diseases/parkinsons-disease/youngonset-parkinsons-disease#:~:text=It's%20not%20common%20to%20see,%2Donset%20Parkinson's%2C%20or%20YOPD.
