Period irregularity?

I am on birth control but was late getting my pills refilled so I started taking them a week after the end of my last period. Usually, when I take my pills late I start spotting around when my period should have started, this time it didn’t happen. Now I’ve been on my placebo pills for three days and my period hasn’t started yet. That means that it’s been over 4 weeks since my last period. Should I be worried? Could I be pregnant?

Pregnancy should be ruled out. If negative, further discussions and exam should be done.
For your security, take a test to rule out pregnancy. Then it is better to start the pills with placebo (28) pills, new pack after finishing your old pack.
Usually this should not be a concern. Consider a pregnancy test if no menses by day 35 (5 weeks) in your cycle and follow up with your doc accordingly.
