If you dont have fever and/or chills, it may just be non-bacterial epididymitis, which I typically treat with supportive underwear and Advil for the next two weeks. If you have a fever or you have any urinary complaints, you need a urine culture and antibiotics. If you have redness, and the Testicle is warm to touch compared to the other side then I would suggest a scotal Ultrasound.
