Nerve pain is radicular and usually runs from the neck or back down the arm or leg
Muscle pain is achy and can mimic nerve pain but usually more in the limb

Signs/sx of NERVE PAIN:
1) Nerve pain may be located anywhere along the path of the involved nerve(s) and be described as lancinating, shooting, stinging, burning. hot or cold, or radiating, and may be associated with sensory sx like numbness, tingling, hyperethesia or dysesthesia in the area of the involved nerve pathway.
Signs/sx of MUSCLE pain include being located in the area of the muscle/muscles involved, and will NOT adhere to any anatomic NERVE pathway. It usually will not have any typical "nerve symptoms" but may be charcterized as achey, tight, pulling, or spasmodic, and may be associated with weakness of the involved muscles.
A good Internal Medicine physician or Neurologst will usually be able to differentiate one from another along with the underlying cause, but when in doubt certain blood tests(ie--CPK) and/or nerve tests(ie-ELECTROMYOGRAM--EMG, or NERVE CONDUCTION VELOCITY--NCV) could be done to clarify the issue.
