It depends on your medical Hx and family Hx. Normally, excruciating pain is with extensive perforation of the bowel. Discuss with a colorectal surgeon and GI physician.
This depends on the cause of the colitis. There are many causes so it's not possible to answer unless some detail is provided on the type of colitis and the circumstances of the condition.
