My brother is HIV positive and also has osteopenia. What is the best/safest course of treatment for him?

He should obtain a bone density test to see how severe his osteoporosis is. Physical activity, especially weightbearing exercise such as walking is very beneficial. If the bone density test shows significant abnormality then he should consult Dr. about potential medication that would be suitable for him. He should also be in close communication with the doctor managing his HIV.
See your doctor. I would start with blood work to check your Vit D-25, parathyroid hormone, serum calcium and phosphorus. Most people simply need high dose calcium and Vit D3. Check blood work again after 4 weeks to check levels.
