Chin reduction surgery, also called genioplasty, is mildly uncomfortable for a few days after surgery. Most patients take a moderate narcotic pain medicine for those days.

Chin reduction is not that painful but it is sore for the first 24 hours.
Chin augmentation utilizing a silicone implant or transferred fat is a fairly common procedure with little pain involved and it is performed under local or general anesthesia.
