The best way to prevent diabetic kidney injury from progressing include
1, Maintaining normal body weight
2. Exercise 3-4 times per week
3. Adhere to diabetic diet
4. Keep A1C less than 7%
5. Maintain BP at or less than 130/80
6. Maintain good hydration
7. Avoid NSAIDs

Ensure you are taking an ACEI or ARB as part of your blood regimen and your proteinuria is driven to undetectable (needs to be checked by your PCM). You should aim for home blood pressures <130/80 and A1c<7. You also should talk to your PCM about starting a daily low dose aspirin and statin to minimize cardiovascular disease risk as you are a diabetic.
Hope this helps get you started.
You likely need to see a nephrologist if you have protein in your urine or your kidney function has declined, even minimally.
Thanks for your question.
