Well smoking with or without a pacemaker is not favorable (hint, hint). But Smoking in general won't affect the pacemaker if that is what you are asking about.

Smoking is clearly not a healthy life choice, but in reference to smoking's direct effect on pacemaker function, there is likely no direct acute issues. Long-term smoking can affect the heart and might eventually affect pacemaker function.
