I am a 42 year old male. My stomach hurts really bad when I don't eat. Why does my stomach hurt so bad when I don't eat?

You probably have a sensitive stomach, which we technically call visceral hypersensitivity. I consider it a neurological problem of the gut. This type of pain tends to be exacerbated by stress/anxiety/depression. Of course, other causes may be present such as a gastric ulcer, whose pain classically gets better with food. See a GI doc for evaluation, especially if this is a chronic problem.
