  • When is dialysis actually required?

    The decision for dialysis is based on land as well as symptoms. If she is at stage 5 and labs are abnormal, e.g., the potassium is high, she has severe acidosis or markers for poor nutrition, or is developing leg swelling/shortness of breath, then it is time to start dialysis. Waiting too long can be detrimental to her health. Also, we don't want to start too early, but start dialysis if indicated by symptoms or labs.   • Can a person with just 20% kidney function survive without a dialysis?

    There are several aspects to this question. Usually, dialysis may not be needed at a GFR of 20, but that is the time an access is planned to ensure that patient can start dialysis in future. Some patients with issues with volume management or persistent hyperkalemia may need to start dialysis at GFR more than 15, but not too many. If she has multiple comorbidities and does not want to do dialysis, then there are options for medical management with dietary modification and supplemental medications. It is difficult to say what the long term prognosis or duration of life may be for each individual patient without actually evaluating them. Data shows that elderly, debilitated patients with multiple comorbidities may not do as well on dialysis as those without these risk factors.   • Does a creatinine level of 5.5 require dialysis?

    It's not just the creatinine number, but accompanying symptoms and abnormal labs that determine the need for dialysis. What is her blood urea level? It seems from the symptoms that she is uremic and may need dialysis initiation. Does she have poor appetite too? Swelling? Nausea? Itching? Confusion? All these symptoms are also concerning for uremia along with the increased sleepiness you described. Please take her to her nephrologist soon. READ MORE

    Protein in urine is concerning. Are you diabetic? Hypertensive? You will need further evaluation, including blood tests for many different disease processes that can cause proteinuria and a kidney biopsy if the protein is significant. You should see a nephrologist soon. READ MORE

    It surely can be a serious situation. Painless hematuria after age 50 does raise concern for tumor/mass, with risk for urological malignancy being higher if you are a smoker or have a family history.   • Is there an alternative to dialysis in case of kidney patients?

    Options at her age include dialysis (hemo or Peritoneal dialysis if she has support at home). Transplant could still be an option if she is felt to be a candidate which will need to be evaluated. If she has a lot of Comorbidities then she will not be a transplant candidate. If she does not want to do dialysis, and has a lot of other chronic diseases, then she may want to pursue only comfort care with medical management.     You are either at stage 5 of kidney disease, which means advanced disease close to needing dialysis. Other possibility is that this is an acute allergic reaction to ciproflixacin due to allergic interstitial nephritis. It will need to be assessed by your exam, blood and urine evaluations.   • Is there any way to pass a kidney stone more quickly?

    There are ways to prevent formation of kidney stones. Once you have a stone evaluation or a 24 hour stone risk profile checked, it will give a clear idea of the kind of stones you have and you can be treated with medications and dietary modifications to prevent this in future. For now, drink plenty of water and limit salt in the diet. Please do get evaluated for a definitive answer. Instead of wanting to pass stones, make a diffference in your outcome by preventing stone formation. READ MORE

    Good control of HTN is essential for prevention of kidney damage. If your primary doctor is unable to manage your BP with medications then you may breed further evaluations to find the cause as you may have secondary hypertension or may just need more aggressive medical management.     This will need to be evaluated with quantification of protein and look for diseases that possibly can cause protein in urine.
