    Dear Reader, Thanks for the question. A tooth that has had a root canal, should not be sensitive as a general rule. If your tooth is hurting when your teeth are being cleaned, something is going on. Why did you need the root canal in the first place? Was it cracked, had a cavity into the nerve? How long ago did you have it done? It can take up to nine months to heal. Was the tooth restored with a crown or a filling? Does it have a post? As you can see, there are several variables to take into account.   • Will a dry socket heal on its own?

    Dear Reader, Good question. Fortunately, dry sockets generally do heal on their own in 10-14 days and are more of a nuisance than anything else. A dry socket is inflammation of the area generally due to the lack of the formation of a fibrin clot or one forming and falling out after tooth extraction. Either way, the wall of the socket is exposed to the oral environment and it becomes inflamed. Over-the-counter anti-inflammatory pain medications such as ibuprofen help. If it is too uncomfortable, your dentist or oral surgeon can place medication in the socket to help alleviate your symptoms until it feels better.     Dear Reader, Throbbing pain is not normal after receiving dentures. This goes for conventional dentures where your teeth are already missing. If you received an immediate denture, where your teeth are removed and the denture is immediately placed at the same visit, that's a different situation. In that case, the throbbing pain could be from the surgery, not necessarily the denture. If your new denture is rubbing against your gum and causing an ulcer, that could be the source of your pain. Most patients (98%) who get new dentures need to have them adjusted. In some cases (I just had this situation), you could have a reaction to the acrylic that the denture is made of. I suggest you return to your dentist to examine both your mouth and the denture. Then he or she can address your problem and hopefully get you comfortable .     Dear Reader, Unfortunately, if you have be truly diagnosed with Periodontitis (gum disease that has advanced into the jawbone), you will need professional care to eliminate it and help you manage it. Is it serious? Periodontitis is associated with greater risk of stroke and heart attack. It will worsen a person's diabetes making it harder to get their glucose levels under control. It is linked to premature and low birth weights in women and ED (erectile disfunction) in men. Periodontitis has been lined to greater chance of Alzheimer's disease. The inflammation of periodontitis is linked to other diseases as well. Once your dentist or periodontist (a dental specialist that treat periodontal disease) and their hygienist gets the situation under control, it will be up to you to maintain and control it so it doesn't return.     Dear Reader, Good question. I would hope your doctor (periodontist, family dentist) who will be doing the procedure will review that with you and provide you with post operative instructions (I always do). Basically, you don't want to eat in anything in the area, or if you do, only very soft foods like yogurt, pudding, soups, eggs, etc. This is because the gum needs time to tighten back down onto the bone. As dentists, especially those of us who make dentures (because we have to set the teeth), we are keenly aware of how teeth should come together and be arranged to make the appropriate sounds.     Dear Reader, Teeth getting a crown are shaped while under local anesthesia. Unfortunately, I don't know what you mean when you say that you are "sensitive" to local anesthesia. If that's the case, I would discuss the situation with your dentist to find an alternative. If it's one particular anesthetic, perhaps your dentist could use another substitute. If it's the epinephrine in the local anesthetic, then your dentist could use one without it. Depending upon the thickness of your veneers, your upper lip can change. If you want to keep your lip where it is, then your dentist will remove about a third to a half of a millimeter of your tooth to place a veneer that is the same thickness. So, the net change would be zero. But, if you want a fuller lip, then veneers can be placed with little or no tooth reduction.     Dear Reader, A bone graft at the time of tooth extraction may help prevent a dry socket. The key to both a successful bone graft and extraction is a healthy amount of bleeding in the tooth socket to form a fibrin clot. Overall, the graft does improve your odds of preventing a dry socket. Of course, do not smoke (if you're a smoker) for several days before or after the extraction and follow your doctor's (dentist, oral surgeon, periodontist) recommendations.     Dear Reader, For gum inflammation, Listerine or warm salt water (1/2 teaspoon to 8 oz of water) and vigorous rinsing may help. So will thorough brushing and cleaning in between your teeth with something like floss of interdental picks. The real question is: what's causing your inflammation. For most people, gingivitis results due to lack of a professional cleaning (prophylaxis). I would suggest if it's been awhile, to see a dentist for an examination, and the hygienist for a cleaning. Replacing tooth structure with a filling reduces the strength of a tooth. But no one would do that to a healthy tooth. We place fillings to replace decayed, diseased tooth structure which is far weaker than healthy tooth. Bonded fillings, done correctly, will restore a tooth to approximately its original strength. If you want to keep your teeth strong, don't get cavities.
