Dr. Cooke an author and physician who is proud to have graduated with honors from IU School of Medicine in 2001 and is now a Fellow of the American Academy of Family Medicine and the American Society of Addiction Medicine, specializing in Family Medicine, Addiction Medicine, and HIV Medicine.

He has received national recognition for the quality and compassionate care he provides by being named Family Physician of the Year by the Indiana Academy of Family Physicians and the American Academy of Family Physicians. He’s also received the Doc Hollywood Award by the Indiana Rural Health Association for his service to rural Indiana, the Indiana State Health Commission’s Award for Exceptional Public Health Service, the Ryan White Distinguished Leadership Award, and the first ever Physician Pillar of Excellence Award by Addiction Policy Forum. 

In addition to operating his rural health clinic, Foundations Family Medicine, he serves as the executive director of the nonprofit community organization, Refresh (F5); as the Medical Director of RCA at Indianapolis, Centerstone Recovery Center in Lexington, IN, and Wooded Glen Recovery Center in Henryville, Indiana. He also serves as Co-Director of the Rural Center of HIV/STD Prevention. 

Dr. Cooke believes that everyone deserves the opportunity to equal access to hope, health, and opportunity. His books, Canary in the Coal Mine, explores this concept through the lens of the 2015 Scott County HIV outbreak. 

Dr. Cooke currently lives in Southern Indiana where he now resides with his wife and six kids.

Dr. Cooke, a trusted family physician and HIV specialist, is the Medical Director of Foundations Family Medicine with offices in Austin and Henryville, Indiana. 

Foundations Family Medicine is a full-spectrum medical facility providing comprehensive care for patients of all ages. Its mission is to help build healthy families by improving individuals’ physical, spiritual, and emotional health by opening access doors to high quality medical services for the underserved of Scott and surrounding counties in Indiana.

In addition to operating his rural health clinic, Foundations Family Medicine, Dr. Cooke serves as the Executive Director of the nonprofit community organization, Refresh (F5), as the Medical Director of Wooded Glen Recovery Center in Henryville, and as the Co-Director of the Rural Center of HIV/STD Prevention.

Specializing in family medicine, addiction medicine, and HIV medicine, he is a Fellow of the American Academy of Family Physicians (FAAFP) and a Fellow of the American Society of Addiction Medicine (FASAM). He is also a member of the American Medical Association and the American Academy of HIV Medicine.

A 2001 graduate of the Indiana University School of Medicine, Dr. Cooke went on to perform his residency in family medicine at IU Health Ball Memorial Hospital, completing his training in 2004.

Dedicated to clinical excellence, the doctor is board-certified in family medicine by the American Board of Family Medicine (ABFM). The ABFM is a non-profit, independent medical association of American physicians who practice in family medicine and its sub-specialties.

To boot, he is board-certified in addiction medicine by the American Board of Preventive Medicine, a member of the American Board of Medical Specialties that issues “certificates of special knowledge” in the specialty of preventive medicine.
