Dr. Slade-Hartman is a trusted family physician with a focus on geriatric medicine. She is in practice with Physicians Eldercare in Winston-Salem, North Carolina.

“I enjoy helping people to control their diabetes and congestive heart failure to decrease the complications of these problems and improve quality of life. I also enjoy helping patients and their families with end-of-life issues so that their family members progress through the end-of-life journey with comfort and dignity” expressed the doctor. 

The clinicians at Physicians Eldercare are committed to providing a high-level of care that is specific to residents of assisted living centers and nursing homes. They share a special interest in geriatric medicine and receive regular training on health and aging. Unlike other groups, they only practice in long-term care facilities, thereby allowing them to fully commit to residents and offer deeper insight into the unique workings of assisted living centers and nursing homes.

Graduating with her Bachelor of Science degree in Chemistry from Wake Forest University, Dr. Slade-Hartman went on to embark on her medical journey. She earned her medical degree from Wake Forest School of Medicine in 1999, after which she went on to perform her residency in family medicine at UAB Huntsville Regional Medical Campus in 2002.

Years later, in 2009, she attained board certification in family medicine through the American Board of Family Medicine (ABFM). The ABFM is a non-profit, independent medical association of American physicians who practice in family medicine and its sub-specialties.

Licensed to practice medicine in North Carolina and Alabama, Dr. Slade-Hartman holds over two decades of experience in her field. She is an active member of the American Academy of Family Physicians.
