    I would need to know if there is any difficulty with urinary stream and if the urine itself is clear microscopically also clear with no evidence of blood or infection. Apparently there was no fluid coming out with ejaculation. Patient could have prostatitis .He would need further evaluation with a urologist. READ MORE

    This could represent prostatitis, peyronies disease, or a stricture of the penile urethral. A flow rate with ultrasound residual along with a urinalysis and culture with possible semen culture and examination should help figure it out. READ MORE

  • Erectile concerns?

    The erectile dysfunction could be diabetic in etiology, but I would check his testosterone level, free and total. If that is okay, then I would consider his trying either cialis or viagra 30-45 min before coitus. If all of that is unsuccessful, then possibly try penile injections. READ MORE

    Patient should have had a prior psa at an earlier age to compare with the current psa at his age of 74. Concern of his current psa would depend on his overall health and whether he has nothing health-wise that would significantly affect his life. READ MORE

    He would need to be examined and questioned carefully about when it hurts and what preceded the pain and what he can do to make it better. He could have intermittent torsion of the testicle. but he would need an ultrasound of the testicles and be examined. READ MORE

Also, a more detailed history would be helpful along with some blood tests and a prostate exam. If all of the above are negative, you could have a trial of jab med. There are also more tests that ultimately would be appropriate if all of the previous evaluation and treatment do not give you an answer. READ MORE
