Her patients are generally living with multiple chronic diseases such as Diabetes, Heart Disease, Parkinson’s disease, Chronic Kidney Disease, Vascular Disease and Dementia. Dr Schneider works with elders and their families to create an environment of safety and wellbeing for her patients.
Dr. Schneider practices at Cambridge Health Alliance (CHA) Primary Care in Somerville and does House Calls to patients no longer able to come to the clinic. CHA is a public safety-net health system based in Cambridge, Massachusetts. Cambridge Health Alliance is a health system serving over 140,000 patients in Cambridge, Somerville and Boston’s metro-north region, CHA patients have access to advanced care for complex conditions at Beth Israel Deaconess Medical Center (BIDMC).
CHA’s operates the nationally accredited Cambridge Public Health Department and community health programs across its service area. It works with the local government and non-profit agencies to provide care for all patients including those who are uninsured and underinsured.
Dr. Schneider earned her degree from the Feinberg School of Medicine at Northwestern University. She completed her residency at Rush University Medical Center in Family Medicine.
