    Sometimes this may be normal. Braces can cause irritation of your cheeks and lead to small ulcerations. If the cause is a bracket, then you can use orthodontic wax to help reduce the friction with your cheeks. If it is a wire, then I would call your orthodontist to see if they can cut it for you.     Your primary care dentist will be able to tell the teeth are coming in crooked or in a less than ideal position. You should take him to an orthodontist because we look at more than just tooth position (i.e. jaw position, potential jaw growth, airway, etc.) The American Association of Orthodontists recommends the first consultation with an orthodontist to be at the age of 7. At this age, we can monitor growth and development of your child's jaws and teeth. There is a big difference between your primary care dentist and an orthodontist. Both your primary care dentist and orthodontist have completed dental school, but only the orthodontist has completed an accredited 2-3 year residency program specializing in Orthodontics.     Finger habits can cause teeth to grow in a more forward position, but more importantly it can negatively influence how the upper jaw grows and develops. If the habit continues into adolescence and adulthood, then the bones may be permanently affected depending on several factors (timing, duration of the habit, the amount of force applied, etc.). If your orthodontist believes the current position of your teeth is causing the problem, then I would ask how this can improve your speech.     Zirconia is a material used commonly in dental restorations (i.e crowns, inlays, onlays, veneers, etc.). In orthodontics, we use tooth colored braces which are not zirconia, but a copolymer.
