  • How painful is a colon resection?

    It depends how we will control the postoperative pain. In any postoperative surgical procedure, pain can be successfully controlled with no narcotics. I always use my Italian education in medicine which is built in the prevention of the pain. No narcotics allowed. I always use anti-inflammatory drugs as scheduled and not as needed. If we use medications as needed, we will always chase your pain threshold. READ MORE

  • How should I prepare for a colonoscopy?

  • Is the colon biopsy procedure safe?

    Any screening colonoscopy provides information on the lining of the colon for the presence of polyps or cancer. Any biopsy is usually safe and it is able to remove the entire polyp or a piece of it, If the size is too big. In the last case, usually a colon resection is advisable to complete the procedure.   • Neck lump question?

    Any lump in the neck should be followed, I would recommend performing a neck ultrasound to check the thyroid gland for possible nodules (papillary cancer of the thyroid can present with neck lymphadenopathy which means enlarged lymph node). Then if the node does not disappear after a month, ask your PCP to order a percutaneous biopsy under ultrasound guidance. I hope my recommendation will help you. READ MORE

  • Adrenal Tumor?

    Adrenal tumors, also called incidentalomas, need to be evaluated for functionality. Several tests are required including blood tests and urine test searching of hormones hypersecretion. Any functional tumor needs to be removed. If it is not functional and it is less than 3 cm you can observe it on every 6 months interval by imaging. Today, I personally perform surgery for adrenal tumors using the robotic platform (DaVinci Xi). Every tumor above 3 cm needs surgical removal. Above 3 cm is the comfortable level of the surgeon to approach the adrenalectomy using a minimally invasive technique. I hope I answered your question with an understanding of what should be done.   • Breast cancer?

    Breast skin discoloration does not mean cancer. At your age it should be very rare. If there is a palpable lesion, I would recommend you to undergo breast US as starting diagnostic imagings. It is important if there is a history of breast cancer in your family. In this case, I would recommend performing the BRCA 1-2 test which is very simple to be done and provides any mutation at the level of chromosome 17. I hope I fulfilled your question. READ MORE

  • For a breast cancer case, is a surgery done first or is the chemotherapy started first?

    Hi, My recommendations are the following: If she is younger than 70 yr old, I would perform bilateral mastectomy with immediate reconstruction by our plastic surgeon. If she is older, a lumpectomy followed by radiation therapy and adjuvant chemotherapy. The role for pre surgical chemotherapy is only indicated for metastatic axillary lymph nodes detected at the time of the original diagnosis or to shrink the tumor in preparation for the mastectomy.   • Is it necessary to remove the entire breast in case of cancer?

    Hello, I am so sorry for your diagnosis at such an early age. This tumor is curable. Based on your young age and the chance that you might have multifocal disease, I recommend you undergo a bilateral mastectomy with immediate reconstruction and axillary sentinel biopsy at the side of the known cancer. This is the way I treat all my patients younger than 65 years old. I have had more than 100 patients treated with this methodology in the last 3 years.   • What are the tests recommended to confirm the diagnosis for skin cancer?

    Hi Melanoma represents 2% of skin cancer. You can use a punch biopsy, a shaving biopsy or excisional biopsy of the entire lesion. If there is melanoma, we need to see the depth of the lesion in millimiters. Above 1 mm, you will need a wife local excision of 1 cm margins and sentinel lymph node biopsy. Above 2 mm you will need a wide local excision of 2 cm margins, sometime with skin graft, depending of the location and sentinel lymph node biopsy. Feel free to contact me with further questions.   • Should I take certain vitamins or supplements after being diagnosed with pancreatic cancer? Which ones?

    Hi , Did you get respected for this pancreatic cancer? If so, all the vitamins can be part of your meals assuming you are on regular diet. I do not believe in vitamin supplements.
