  • Is a fatty liver anything to worry about?

    Fatty liver is a very broad spectrum term. NASH is non-alcoholic steatohepatitis. There’s idiopathic, familiar, associated with multiple different endocrine disorders. Is a great deal of information that would be needed to answer that question appropriately. I would tell you with as frankly as possible with your labs ultrasounds x-rays CAT scans MRIs biopsies potentially are all needed To answer that and your gastroenterologist or specialist or subspecialist or even your general practitioner would have that information for you and able to go over that.   • How long does it take to recover after liver surgery?

    Depending on how much is being resected, what type of medicines you’re on, and will be discharged with, any other pre-morbid or pre-surgical medical conditions, allergies, medication sensitivities, etc. of all been completed and you have been given the greenlight by your primary care provider. As well as, any and all of you are treating providers with their suggestions for treatment preoperative the postoperatively. And on signing your consent form you’re normally get a packet of what to expect and that goes over it in very simple basic terms which are very easy to understand and apply. If you’ve not received any of that I would contact the provider who is going to be performing your surgery and ask for the aforementioned and or of the questions I stated to you of what I would need to answer your questions.   • My son was in a car accident, what to do?

    I would take him to the different emergency department and request a neurologist, a neuropsychologist, and EEG, a CAT scan of his brain and neck, and also a psychological evaluation with a complete metabolic profile panel and the works. He needs to be admitted to find out what’s going on. He’s passed the outpatient stage and needs the inpatient stage. And put your foot down as his mom and make sure that he gets admitted so he has the right diagnosis comfort and care before he leaves. My opinion is three strikes and he’s in, not out.   • Can my breathing issues be related to a swollen tonsil?

    The quick answer is yes. But if you look at your uvula, I caught the little punching bag hanging down in the back of your throat, on the right hand side of the picture or the right left-hand side of your mouth you’ll see that there seems to be a little bit of inflammation. Secondly behind your tonsils appear to have difficulty describing what I’m seeing other than the three combined would definitely be a differential in the diagnosis of why you’re snoring while you’re sleeping especially if your a mouth breather. I would speak and or see an ear nose and throat specialist to get a better evaluation and see if there is any other things that could result in it. Lastly a low lying uvula/palate would also hinder some breathing and we would result in or could result in, snoring. I hope that was of assistance and I wish you well.     SSRIs would be a choice, yet there are multiple different classes of anti-depressants.   • What anesthetic agent should be avoided in geriatric patients?

    If you turned an aspirin bottle around and looked at its mornings you would never take an aspirin again, yet it’s prescribed all the time. Each patient and each person has a different effect with different medications dosing and timing. Throw and dementia, and now you put a wildcard in that your cards. Whatever is prescribed by the treating provider, is most likely the logical choice based on the symptoms. If there are adverse reactions, you need to put it down on a date, time, and explain what happened and how long after the medication was ingested.     I need to explain to your parents what is going on and I need to see a gastroenterologist. I want to make sure that you don’t have ulcerative colitis, your old pal syndrome, Crohn’s disease, a diverticular problem, and a multitude of others. When your stool turns dark like that you want to make sure there’s no blood in it. Do you want to be evaluated by a specialist? Yes, you should see a doctor, the differential is far and wide, and we’re not even talking about your gynecological history as well. So with that in mind, let your parents know what you need to do and where you need to go and that is to a gastroenterologist to get a better evaluation and also blood test urine test stool exams to make sure and find out what it is and more importantly for you what it’s not.   • Covid/sinus issue?

    Thank you very much for the very detailed description of your history. The only thing that I would say right off the bat is what was your influenza swab? Are you being repeatedly in contact with influenza? If so and people that you were in close proximity with have Covid influenza and Covid equal a superinfection that you definitely don’t want. And you wanna make sure that that’s the case. Why a chest x-ray a sputum sample blood test all of those things I haven’t been done or tested I can’t answer is I’m not you’re prescribing physician. So I would make sure that all the necessary tests need to be tested and then you could work from there. It sounds like a sinus infection and Levaquin 500 mg a day times 10 days if you take it off why is it clear it all up. And pneumonia, bronchitis, and a multitude of other illnesses. So, get the necessary test by the appropriate physicians, nothing wrong with starting on that medication, over-the-counter medication I would recommend would be nasal saline, Afrin every once in a while but no more than 3 to 5 days in a row, and hot showers Netty pots old school remedies worked because they worked.     Bedbugs, spider mites, a multitude of other small pestilent individuals that just like the bike and go and gone. The health inspection department and oh God I don’t even know who else… That needs to go over there because that’s unhealthy living conditions. I can only imagine the last time your air filtration system was actually taken care of. Do you actually know? Because you’re probably breathing dirty rotten stinky moldy parasitic Viereck air. Other than that I know I don’t have much more to offer other than go to the ER and they gonna put you in a special room by the way, but that’s to protect everybody from what you might have.   • Mystery illness?

    Yes, you should seek a second opinion. But there are many questions based on your conversation that I would want to know in order to direct you in the correct way and manner and provider. Examples would be, vitamin D level is it too high or is it too low Are you taking vitamin D3 supplements and at what dose, hypothyroidism is your TSH T3 T4 all of the above, Are you on levothyroxine and what does? When do you say elevated liver enzymes which ones? Did they do pancreatic enzymes? Do you have bowel symptoms and irregularities and difficulty with digestion? These are the things that you would want to talk with a provider about face-to-face while you can get an appropriate examination in order to follow through with those. The question that comes to my mind is doing you have a MEN syndrome or a degree of and or subset of that.  So to come back full circle again I would definitely seek a second opinion, and bring all of the necessary information labs tests timing, etc. to that provider so they can be fully informed to do the right tests and exams without the shotgun approach. Meaning shoot in the air whatever fell down that’s what you were aiming for. That’s not the best way to practice medicine. Focused but making sure you have a wide differential and eliminating those differentials as you go through the process.   • Rectum discharge?

    Depending on what type of medication your mother is taking could result in your description. If she is no longer on chemo, radiation, or any other type of medication I would make sure that I go and take her in to get a valuated because there is a problem. If she can’t hold it in it could be muscular which could be meaning neurological, which could be anywhere from the spine downwards. It’s not always the most comfortable or easiest thing to deal with and to get to a provider earlier would be much better suited than later. It very well is nothing more than viral gastroenteritis or viral G.I. symptom and resulting in that, or let’s say bad food is eaten, any number of things including good for your food it just increases movement through the bowel tract. Getting her in to be taken care of in a valuated with a provider is important and also noting her rectal tone Will be able to tell you about her neurological ability in order to control her anus and rectum from having leakage until she can get to a bathroom.   • Pain under my left rib and test results?

    Based on your description an area of your discomfort as well as the medication you’re on mites be resulting in mild pancreatitis. If you are using alcohol during taking that medication it will get worse, or if you’ve been on the medication for a significant period of time you might have developed a tolerance and the pain medication is no longer working. If you’ve stopped taking the medication abruptly without a taper you might be experiencing withdrawal symptoms mildly. Speak with your provider about where your pains are and also did they do any pancreatic enzymes. A stool test for occult blood is also reasonable to ensure that you don’t have any gastric irritation because of your medication that you’re taking for your migraines. Your symptoms may all be described by the medication that you are currently on. This is something that needs to be discussed with your prescribing provider for further evaluation.   • Can this be normal?

    There are multiple issues that you have drawn attention to and I will try to do my best to make sense of it for you. Below iron is always an issue because it’s very difficult in order to make red blood cells without iron. Calcium is also very important as part of a co-transport system into the cells, the creatinine and the other lab abnormalities can all be a result of the fact that you’re having an inability in order to produce enough hemoglobin because of a lack of iron. One other thing that wasn’t mentioned which could be very important is your menstruation, cycle, and how many pads you go through, and how long it lasts. That is a very common way for women to become anemic through excessive menstrual bleeding. Therefore, I highly recommend that you see your provider and ask about your swollen lymph nodes because if that is the case, I would recommend a fine needle biopsy to find out why your lymph nodes are swollen, as well as possibly a bone marrow biopsy to make sure that your bone marrow is functioning properly. Therefore seeing a hematologist would be beneficial for you to help work through the multiple issues you have. It’s normally easier to make one problem with multiple signs as you described as opposed to having multiple diagnoses because of abnormalities.
