    Profession now "Physician Associate". Very much as an MD or DO. I have worked rural clinic, inner city ICU. Urgent Care. My typical day, currently Internal Medicine Hospitist. My responsibilities include all admissions into the hospital. Evaluate patients in Emergency Department for appropriate route and establish foundation of care while in the hospital. Typical day, I walk into my office for pass down of patients from the following shift. Check what patients I need to see immediately. Then round to ED for patients that need to be admitted or transferred to appropriate care like a specialist such as a surgeon or cardiologist. I preform a complete history and physical exam. Order type of admission, appropriate labs and diagnostic tests, interpret. Dictate entire admission. End of shift I pass off to on coming shift. Basically this is in a nut shell of my typical shift.   • What is the role of a physicians assistant?

    Correct title is Physician Assistant, soon to be Associate. A PA is usually a person that has some type of prior medical training ie..paramedic, nurse, xray tech. A bit of history, during the Vietnam War many corpsman, medics were returning home and had extensive training in the battle field. Duke University Medical Director recognized this and did formal training in a fast trend model of mini medical school. Thus, the result was the PA. The PA does much as a MD or DO. They are trained in extensive physical exams, lab, diagnostic radiography and interpretation. A diagnosis. Usually, common diagnosis seen in hospitals, ER, outpatient clinics. This is supposed to free up the Physician to concentrate on more complex diagnosis. However, over the years PAs have been gained more acceptance and allowed wider range of responsibilities. It is a highly competitive and respected profession. Feel free to ask me more. I, myself, was a paramedic and army medic.   • My infection isn't settled after I've been on antibiotics. Why is this happening?

    I recommend return to the person that Rx'd the antibiotics. One possibility is the bacteria is resistant or not the right antibiotic for that particular bacteria. Maybe a virus. But, definitely, follow up is important. READ MORE

    More than likely design and materials of the thermometer. Flawed calibration. Different readings can occur from site taken, ie...forehead scanner, oral, rectal, ear. READ MORE

  • When does a fever become dangerous?

    99.5 F is considered a fever. It's the body's natural response to an infection, most times. Younger than 3 months old should be seen by health provider. 6 months to two years old if the fever lasts more than two days. If greater than two years old and listless, irritated, vomiting or in intractable pain they need to be seen by a provider. For "colds" , it's a virus. Usually, self limiting. Hydration (pedilyte), Tylenol, bed rest. It is very normal to have a small fever with a cold. If you have a "gut" feeling, go with it and have your nephew checked out.     Over the counter med that can help...anything with nystain. This sounds odd but there is a nystain vaginal suppository that you can get and use similar to a throat lozenge. Otherwise, prebiotic and a trip to clinic for a prescription. READ MORE
