There, she played a pivotal role in designing and executing high-performing clinical care delivery models, which included innovative home-based and mobile care solutions, virtual care, and unique primary care models in value-based agreements. Her work has been instrumental in enhancing patient outcomes and ensuring that healthcare services are delivered efficiently and effectively to those in need.

In addition to her consulting work, Dr. Berchuck has served as a home-based primary care provider and an Attending Physician at Brigham and Women’s Hospital, where she applied her clinical expertise to patient care.
