You say that you survived COVID-19 in November 2019. Did you mean November 2020? The vaccines are no longer experimental, although they are used in this country under an “emergency use authorization." This means that they have not gone through all of the analyses typically required under typical conditions. But the “experiments” have been done. We know they are effective in preventing infection and especially severe illnesses. While infection may still occur after two doses, the illness is much reduced. There is a cost: some persons experience feeling lousy for a few days. Over time, rare complications may occur that we have not recognized. But Covid-19 is often a severe disease. No vaccines are perfect, and we will learn more about how long protection lasts and which vaccine is best. That is true. If I were your doctor, I would urge you to be vaccinated now. In my opinion, you are served by vaccination against a vaccine preventable disease. I am an older physician, and all of my family has been vaccinated, including two who had Covid-19 infection.

Condom slipped during sex with an escort, what is the HIV risk?

Assuming your escort was honest, your risk appears to be very low. HIV infection may cause no or minimal or severe symptoms. The simplest approach for you would be to be tested. The latest versions of HIV tests are quite sensitive, and if negative 3-4 weeks after the incident would confirm no infection. Remember that Covid-19 is not considered a sexually transmitted disease, but such an encounter as you describe would represent a risk if your escort was infected. You might want to be tested for COVID-19 too. The same for other STDs.

Mutations may be neutral, negative, or positive for the virus. The mutation may have effect on the virus. The mutation that have negative effects generally simply disappear. Those with positive effects “help” the virus. This positive mutation may enhance the virus’ ability to reproduce and maybe to cause more disease in the infected person. That may or may not be helpful to the virus which basically wants to reproduce and spread. So, there is no way to know. Ideally, the virus would do best by infecting without causing much disease, but spreading easily - as it does among children.

I think the evidence is that she does not have HIV infection. She seems to have an immune deficiency as invasive Candidiasis is very unusual. Three tests would help: an HIV RNA PCR assay (to see if she might have HIV), T-cell subsets to check her cellular immune status, and immunoglobulin subsets. There are other immune deficiencies.

I think that you should be evaluated before any treatment:
-Check pulmonary function to be sure lungs are working
-If lungs are working, then a cardiac evaluation to see if your heart is working properly
-If lungs and heart are okay, then work up the autonomic nervous system
-Before then, check thyroid for hyperthyroidism, complete blood cell count
-To exclude anemia and I suppose changes in white blood cell counts

This may be expensive, but given the interest in defining complications of Covid-19, it may be available and not so expensive.

You do not specify what treatment you received. For early latent syphilis (no signs of syphilis, but the laboratory test), that is, syphilis acquired in the prior 12 months or less, the treatment would be 2.4 million units of syphilis one time. If the latent syphilis is more than one year's duration - or you do not know the duration - the treatment is the same, but given three times once a week apart. Your serology is improving appropriately as expected for successful treatment.

You do not mention anyone but your father and you, though you say “us.” At 13 days out, you are unlikely to transmit (assuming you are immunosuppressed). I think that he can bring the groceries to your door or patio, not have physical contact with you. If there are others in the house, keep masks on them and away from him. He is expected to be protected from serious COVID-19 disease, and probably from infection, but no need to risk transmission.

I do not fully understand your question. If you have been living together as man and wife for the past year, neither are sick, why should either of you worry about catching SARS CoV-2 from the other. Am I missing something? Get the vaccine tomorrow. It will not protect you for two weeks and if you receive either the Pfizer or the Moderna vaccine, you will not be fully protected until 2 weeks after the second dose. There is no dormancy with the SARS CoV-2 virus, so the only concern for the future is that the circulating virus might mutate and no longer be susceptible to the immune response your vaccination induces.

So:
-Do not worry about the kiss
-Get vaccinated to morrow
-The protection will be present in two weeks from now and fully protective two weeks after the second dose.
-Watch the news about any change in the future stages of the epidemic.

Best of luck.

Working out past Covid infection?

10 days from onset (and 3 days after improvement). Myocarditis is infrequent, so as long as you are not doing vigorous work, I would not worry.
