Chronic arm pain

I am 43 years old, and a semi regular rock climber. I have been experiencing what I believe is bicep tendonitis at my elbow for a few years now. No amount of rest time, stretching, or rehab seems to help.
When I was 24, I had a fairly serious staph infection in the same arm that definitely felt like it affected the tendon in my arm. I was unable to straighten the arm until I got it treated.
My question is: could there have been some permanent tendon damage from an infection 20 years ago that is affecting me now?

Yes. Residual scar from erosion of tendon and stiffness. What is source of infection?
Yes but overuse tendinosis of the distal biceps is more likely.To clearly identify the pathological anatomy get an MRI.Make sure the Radiologist who reads the MRI knows your history
Previous Infection in tendon or joint can cause damage to the structures. It depends on severity of infection and length of time it lasted. If pain is recent and there was no pain in between the period of age 24 infection episode to current age, then it could be different reason. If there was on and off pain since that episode of infection since age 24 then there is possibility that infection might have caused some damage. I would suggest to see your doctor for evaluation.
I doubt that the current pain is related to the old situation. Good luck coming to a diagnosis of current symptoms. Likely you will need office evaluation and imaging such as elbow x-ray/MRI.
