Why over the past month have I been getting these flaky red spots all over my back, shoulders, face, and stomach now? I moisturize and the flakiness reappears, sometimes they itch.

If the sites do not clear with hydrocortisone cream, best to get checked by a local dermatologist.
Hard to say. Tinea versicolor comes to mind though. You should see a dermatologist for an accurate diagnosis - this is best accomplished during an in-person consultation.
