Heart issue?

My 83 year old mother has some swelling in her shoulder muscle and some pain. Should we take her to the emergency room?

Seems something for the PCP.
The swelling can have many causes:

1. Varicose veins
2. Weaken heart function; if that’s the case, she should be short of breath
3. Clot in the veins (deep vein thrombosis); this can be diagnosed with an ultrasound.

If she is not short of breath, not having chest pain, and the leg is not painful, seeing your primary care doctor is best.
