Orthopedic Surgeon (Orthopedist) Questions Physical Therapist

Knee fluid?

How would you treat someone who didn't injure their knee but has vast swelling around the knee? Yellow fluid, synovial more than 50ml. I had this before and exploratory surgery, arthroscopy, didn't find any abnormalities. What I think cured me was the surgery itself, saline.

Most important test on fluid is CELL COUNT
Less than 2000 cells - likely non inflammatory
Greater than 2000 / inflammatory cause
consider Gout, Pseudogout, Lyme Disease, rheumatoid arthritis, psoriatic arthritis,
This is likely to recur but wish you well
take the fluid out and send for several tests for such as crystals ( gout) or bacterial cultures ( infection). You may have synovitis so need to have rheumatoid arthritis ruled out with blood test.

I would suggest an analysis of the fluid to ensure there are no abnormalities related to infection, inflammation, Gout, or Rheumatoid arthritis. I would also suggest an MRI to evaluate the cartilage and supportive bone and ligaments.

The picture of the knee shows a significant effusion. Typically, drainage of the excess synovial fluid in combination with analysis of the synovial fluid to determine the cause of the arthritis and synovitis. A return to your orthopedic surgeon is warranted. The surgeon or a rheumatologist can direct you appropriately.

Good luck,
