Hysteroscopy vs. Endometrial Ablation?

Wife is 38, anemic, otherwise healthy. Two children, both vaginally and no issues with babies or mom. Takes no medication daily. Should take iron, but doesn't due to poor tolerability. Has been anemic for several years but with a couple infusions per year, stayed fairly within normal guidelines with her blood work.

In the last two months, she's had two transfusions and three infusions (Injectafur). None of them have raised her H&H, and/or her RBC Panel?? The three RBC numbers, more than a point or two. Now she's getting shortness of breath, tachycardia under very minimal workload and extreme lethargy. She works full time and really is a trooper. Talking Hematocrit of 2 and 3. ER Doc said he couldn't believe she was sitting up talking to him. Comes in from work and just collapses onto the couch and would stay there till morning if she wasn't such a busy body. That's the background.

Finally talked her into seeing gyno for her very heavy bleeding every month. After the usual exam, doctor performed a vaginal ultrasound which she said was unremarkable.

Why does a hysteroscopy under general anesthetic when there are other options seem much safer? For me, I would avoid general unless absolutely necessary. It just seems like an unnecessary risk, the information I am working with anyway.

Also, I would think hysteroscopy would be much more expensive than an in-office procedure. Cost of OR and supplies, ancillary staff, etc, etc.... Recovery times longer with hysteroscopy as well. What am I missing? On one of the EA websites, a more popular one seems, they show another doctor in the same practice as offering the procedure, in office, local anesthetic. Any thoughts would be greatly appreciated. Curious mostly. My wife seems A-OK with it.

If the uterus is a normal size, ablation of the Endometrium or having a long-acting IUD (e.g., mirena) gives the same good result.
