Suggest a urologist. They handle cases of E.coli resistant strains to antibiotic.
UTI in men are unusual and can be sign some blockage in the path of urine, due to for example an enlarged prostate or scarring in the urethra, leading to incomplete emptying of the bladder. E.Coli, which is resistant to some antibiotics, is not uncommon. If Keflex is working for you, that's a good sign. If you are prone to infection, you should have a work-up to see if there is an underlying cause, including an ultrasound of the kidney and bladder, possibly a CT scan, and a cystoscopy (look inside your bladder). You may also benefit from a limited course of urine sterilizers, such as Urex, or even suppressive antibiotics if this is a very frequent problem.
As for the spots, I can't think of a connection.
