Yes implants can break sometime, but it’s depends on what type of implant or depends on size of implant they use.
Also how you chew your food and what kind of jaw movement you do.
When implant breaks it’s depends on all different conditions, just like how you breaks your own tooth.

Usually you shouldn’t have any pain at implant posited area, but you may have soreness at gum area, not on bone area.
Soreness may last about one week.
Any surgery you have you always experience some pain after the procedure, but it usually goes away in few days and feel soreness about another week.

It’s not necessary to have CT scan but if it possible it’s better to see how implants placed good or not with CT scan.
We can use regular x-ray pictures to see position of implant placement.

You can get dental anesthesia, but depending on the condition of your decay, you may not need anesthesia, and also depending on your health condition, it is not always necessary you get dental anesthesia. In my practice, I don’t usually give anesthesia on small fillings since it doesn’t give you pain with my technique. I don’t like to use medication unnecessary to every patients. I try to avoid using too much medication on every patient as much as possible.
