Does a non-cancerous adrenal tumor need to be removed?

The answer depends upon the size of the adrenal tumor. If less than 4 cm, most often these are monitored with ct scan or MRI over several months. If the adrenal tumor is functional (e.g. it is secreting too much of a specific hormone) than it should be considered for removal.
Adrenal tumors, also called incidentalomas, need to be evaluated for functionality. Several tests are required including blood tests and urine test searching of hormones hypersecretion. Any functional tumor needs to be removed. If it is not functional and it is less than 3 cm you can observe it on every 6 months interval by imaging. Today, I personally perform surgery for adrenal tumors using the robotic platform (DaVinci Xi). Every tumor above 3 cm needs surgical removal. Above 3 cm is the comfortable level of the surgeon to approach the adrenalectomy using a minimally invasive technique. I hope I answered your question with an understanding of what should be done.

2. is it larger than 3-4 cm? - YES
3. Do you have access to a surgeon that can do the operation at 3-4 cm? - YES
4. it is inert, smaller than 3 cm and you don't have access to a minimally invasive surgeon like myself, than the answer is no, you need only surveillance with scheduled CT scans.
