Dull testicle pain?

I am currently seeing a urologist for chronic prostatitis and being treated with prostate massages and antibiotics. But I wanted to hear another opinion from another doctor. For the past year, I get a dull pain in my testicles continuously that can range from a scale of 3 to 10. Pain intensifies during any kind of sexual activity, but more so during masturbation. Both testicles have swollen at times. I do get pain in my abdomen and under my testicles. On the left side of my scrotum, there is a puffy lump. It is not painful but concerns me. Ejaculation can burn as well as I get the need to urinate. I do need to urinate at night it seems. Since the doctor has begun prostate massage, I have noticed my scrotum isn't hanging as low. Wasn't sure if that was normal.

My concern may be related to retrograde vasitis means sometimes urine may enter the vas and cause irritation and some inflammatory changes. You need a close examination and maybe an ultrasound of the testis and the cord .

Chronic prostatitis is in more recent times an antiquated diagnosis - and what most of us in urology would tell you is you have CPPS which is chronic pelvic pain syndrome - I really don’t treat patients with prostate massages and antibiotics - do some reading about CPPS and you’ll see!

Most scrotal discomfort is really just from a guy doing something exerting, but under the circumstances, you should have a scrotal ultrasound performed - just to be sure you don’t have any uncommon stuff going on inside your scrotum - it’s also possible you have a left varicocele.

The issues that you bring up in the last three sentences of your series of questions are irrelevant to your general health and are very non-specific things that you need to try to do you focus on and move forward and just monitor.
