Early Miscarriage or Late Period?

I’m in a predicament. I had protected sex (condom, didn't pull out) with my ex-boyfriend during my predicted fertile/ovulating days. I was fully convinced I was pregnant (mother's intuition?) But I've never been pregnant before. Yet, I even grieved when I started bleeding a lot, thinking I wasn't pregnant after all.

My symptoms:
Entirely skipped my period
Nausea
Nearly threw up
Passed gas frequently
Extreme cramps in the uterus region
Lower back pain
Extreme rectal/anus pain
Very heavy bleeding
Bloody "tissue" or "clots"- bigger than normal, and rare that I have these at all
Tender and fuller breasts


Here's the story:
On October 3rd I had sex with my boyfriend, I'm was estimated to have my period around October 21 through to the 26th. However, I missed it entirely, which is very very rare for me. While I was waiting for my period I felt dull and constant nausea, going on my phone or on a car ride made it so much worse. For three consecutive nights, I had the worst cramps and lower back pain I've had since I had appendicitis. I almost went to the emergency room because I thought it wouldn't end, but it usually did within 30 minutes. My rectum also hurt a lot during this time, like a very sharp pain. Sounds weird, I know. I tried two pregnancy tests, one on the 22ed and one on the 25th. But it could have been too early for the amount of hCG I had at the time to be registered. Then it just stopped and I started my period two days after my predicted period was supposed to stop, which is very rare.

What do I do? It’s been 12 days since I stopped my period and I haven’t had any complications, symptoms, or irregularity since then. Will the hCG still be in my system for me to be able to test myself? Should I tell my general health care doctor? Should I assume it is a miscarriage and grieve it as such? Or does mine not count as one?

It is difficult to tell. If you see an OB she may want to test you but it is late enough that it could be negative.
Based on your timing and the dates that you recall, it’s quite possible that you were pregnant as condoms can be outdated and leak, even without having a tear or pulling off so always check the expiration date. Your extreme pain, esp feeling it in your rectum can mean a few things: it could mean an ovarian cyst that would sit like a water balloon in your pelvis and could sit on your rectum. Ovarian cysts can go away or can rupture; but are painful while they are larger than they should be. Rectal pain can also represent an ectopic pregnancy that can start to bleed with pressure in your rectum or you can miscarry an ectopic and it will land its products in your pelvis and can end up around your rectum. Then once you ‘miscarry’ the ectopic, your cycles will resume.; but the bleeding can cause scar tissue in the abdomen so it can be a troublemaker. Severe pain in your pelvis or abdomen is rarely going to have a happy ending; it usually means you need to have medical attention so don’t be afraid to contact your doctor; scar tissue in your abdomen or pelvis can cause trouble down the road so get checked out.
You can test, but with serum, this far out, it is possible that it will be neg or it may come back with a low positive result. It is hard to say it was a miscarriage without a positive test. It could also be a late cycle where there can be a very heavy period.

Most women will have 1-2 abnormal cycles per year, whether they skip a cycle or have an extra cycle. This could be that scenario or you likely had a miscarriage. The only way to tell was to do a blood pregnancy test around that time of the missed cycle.

Over-the-counter pregnancy tests are pretty accurate in detecting hcg 3.5 to 4 weeks after your missed period. All the symptoms you just described also could be due to a ruptured ovarian cyst including nausea. It is a good idea to see your primary care physician or OBGyn, may be get a pelvic ultrasound to make sure everything is okay. They can also run a pregnancy test again which may be negative, but worth checking.
