Hemorrhoid large excruciatingly painful?

I had hemorrhoids the size of a walnut this past weekend. It is the third time in the past 6 months I have had this excruciating problem. I have never had them before. After they burst I just go about my business. Yesterday, the darn thing burst as I was on my way to go to the hospital, thankfully. However, unlike the past two times, this one keeps seeping yuck into my underwear. I examined my rear end with a mirror and flashlight I noticed a hole in my rectum. How concerned should I be and what should I do to prevent infection?

This is likely an abscess that drained itself. But there seems to be an underlying fistula tract causing it to recur. You will need evaluation by a colorectal surgeon or a specialist to diagnose and fix the problem. Anal fistula can only be repaired surgically. I hope this is helpful.
