How bad is a cavity if it hurts?

I am a 38 year old male. I want to know how bad is a cavity if it hurts?

Most likely the caries lesion has reached the pulp (where the nerve resides) and will need a root canal therapy.
If a cavity hurts, it probably means it is very deep and close to the nerve. If the nerve is infected, the tooth may need to have a root canal or be extracted.
If a cavity hurts, more than likely it will need a root canal.

If the cavity is close to the nerve, you may need a root canal, but first, try a setitice dressing before root canal therapy.
Without examining you in person, I cannot say for sure. But as a general rule, if your tooth is already hurting, then the cavity has already progressed into the nerve space of the tooth. This may end up needing a root canal if not addressed quickly.
By the time you feel pain it may be quite deep. Root canal is possible. Go see a dentist immediately for a proper evaluation with radiograph.
