It sounds like you should be evaluated by a Board-Certified Fellowship-Trained Hand surgeon.
I'd be happy to see you.

Go to an urgent care center or an emergency room. As soon as possible. Especially if the skin has been Violated or cut?
Have a physician relocate it as quickly as possible, then get x-rays to check if you broke anything in the process of dislocating it.
Acute is reduced and splint Chronic needs surgery.

The simple answer is that it needs to be reduced. If it is an acute injury, this is simple and straight forward. If it’s more than a couple weeks old, it will likely need surgery to put back in place. I would recommend you see an orthopedic hand surgeon.
A dislocated finger joint needs to be put back into the proper position. If it happened recently, sometimes it can be done externally. If it happened some time ago, sometimes it requires surgery.
