I have a heel spur and want to get a cortisone shot. How long does it take for a cortisone shot to work on a heel spur?

It depends on the type of cortisone that is used. There are rapid acting and slow release. In either case, it is not instantaneous. It is a slow gradual process that can go away in days or take weeks depending on the severity of the pain. Usually there are noticeable decreases in the pain, usually over days.
Some people get immediate relief from heel pain with a cortisone injection. Some people require more than one.
A cortisone shot injected into the heel for conditions such as plantar fasciitis typically takes a few days to start working. Most people may experience relief within a day or two, while for others it might take up to a week or two to feel significant improvement. It's important to note that the effectiveness can vary depending on the individual's condition, the severity of inflammation, and other factors. Additionally, the duration of relief provided by a cortisone shot can vary from person to person, ranging from weeks to months.
It typically peaks at day 3 to 5 and can provide leave for several weeks or even several months, sometimes a second or third injection may be necessary, however, in order to target the exact pinpoint spot.
Cortisone is a temporary internal band-aid to help decrease inflammation if the "heel spur" pain does not go away with normal stretching and change in shoe gear and possibly soft inserts. The heel spur does not hurt as it is secondary to the tearing of the plantar fascia away from the heel bone causing plantar fasciitis and subsequent infracalcaneal neuritis. I only recommend 1 steroid injection and it should decrease the pain within 1-2 days. If this does not last long then I recommend a non-surgical procedure called Orthotripsy to resolve the issues. Radial ESWT is a good form of physical therapy but requires multiple sessions whereas Orthotripsy is a one time procedure for the resolution of plantar fasciitis. I hope this answers your question.

Each patient can respond differently to injections. Some patients can feel relief within 1-2 days others can take up to 7-10 days.
The injection is treating the soft tissue inflammation, not the bone.
After receiving a cortisone injection it takes 2-3 days for the injection to start to take effect. Some have taken a little longer.
